On page http://iesdp.gibberl...ts/cre_v2.2.htm the item slot description is wrong.
Offset 8 is not an usage count, but a counter for temporary items.

So:
Offset Description
8 - Timer for temporary items
a - Amount/Charges 1
c - Charges 2
e - Charges 3
10 - Flags
