I used the saved game provided with the mod and LT5 never happened. I beat the trial itself, then went to the temple. Some of the companions showed; Khelgar, Qara, and Casavir. No Bishop. If the LT was to happen anywhere I figured it would have been in the temple. Has anyone else had troubles with this LT?

In the temple, the man with higher influence shows up, in your case - Casavir rather than Bishop. With the way stuff is scripted some talks will not occur.
NWN2 is not the most stable game in the world, hence the corrupt save. Try replacing whichever files you are having troubles with from an uncorrupted save.
